IAPMO uses the information in this application to determine your qualifications and to assure that IAPMO technical committee appointments are made in a way that ensures that committees will contain a fair balance of interests. Instructions for completing this form:

1. Read each section carefully before completing.
2. Tab through to get to a new section of the form.
3. Fill in the form completely. DO NOT LEAVE BLANK SPACES. If a section does not apply to you, indicate “N/A” (not applicable) or, if you need more room than is allowed, you may attach a separate sheet of explanation as a “Word” document or in PDF format.
4. If you are applying as a representative to an organization, you will need to supply a letter of authorization. It must be submitted on the organization’s letterhead and signed by an officer of the organization. You may fax this document to IAPMO at 909-472-4157, or you may mail it to IAPMO Standards Council Secretary, 5001 E. Philadelphia Street, Ontario, CA 91761-2816. Your fax cover sheet/transmittal sheet should indicate that you are/have submitted your application online, including the date submitted. This letter may also include information requested in Section 3, parts (a) and (b). The following is a SAMPLE of suggested wording for the authorization letter.

SAMPLE: We, (organization’s name), authorize (your name) as our representative to serve on the technical committee. We will support (your name) for his/her time and travel expenses during his/her service as a member of the technical committee.
